Sunshine Silver Mine in Idaho where they had a big fire in the 70's with 100 odd killed. It recently re-opened, only to close yet again in the last few weeks.

It is believed that the Markham Colliery Explosion of 1938 was caused when some tubs ran uncontrolled down an incline and were derailed spilling coal dust into the air. At the same time they cut an electrical cable causing a spark which was the source of the ignition of the coal dus
