The framing would not bear the whole weight and the pulley square was torn out and that broke the rope. This experienced man ought to have known better; but at the same time, the framing and tackle were certainly defective.

FLETCHER H. - 1856 - 20/06/1956 – Age 14. Old Park No. 20, Lord Ward, Dudley, Worcestershire. Breaking of a single link of chain. 8 killed 6 instantly and two died after. They were ascending in one skip and had nearly reached the top when the chain snapped and they fell 20 fathoms down the shaft.

FOWLER Francis (injured) - 1851 - 12/12/1851. Soundwell, JJ and J Whittuck, Bristol, Gloucester. Haycock boiler burst. The engine had hauled up a journey of men and sent down a cart of timber at Soundwell Lower Pit when the boiler exploded. Thomas Waller, the younger, who was attending to the resultant fire was killed. Francis Fowler was severely scalded and taken to the Infirmary.
